Choice of Surgical Procedures for Thoracic Ossification of the Posterior Longitudinal Ligament
نویسندگان
چکیده
Thoracic myelopathy caused by ossifi cation of the posterior longitudinal ligament (OPLL) is rare compared to cervical OPLL, even in the Japanese population. However, once myelopathy appears in patients with thoracic OPLL, it is progressive and often leads to serious paraplegia. In such cases, surgical treatment is required because conservative treatment, such as immobilization by a brace, is not effective. The mechanical stability of the upper and middle thoracic spine is greater than that of the cervical spine. Therefore, with thoracic OPLL, the static factor—compression of the spinal cord from the anterior aspect—is a major factor for the progression of myelopathy rather than the dynamic factor. When myelopathy is mild, both surgeons and patients sometimes hesitate to undertake surgery because of the potential risk of the neurological complications that might occur. As myelopathy progresses, the risk of surgery increases, which presents a dilemma for both patients and surgeons.

Simultaneous Diffuse Idiopathic Hyperostosis, Ossification of the Posterior Longitudinal Ligament and Ligamentum Flavum
Background and Importance: A 60-year-old diabetic man with simultaneous diffuse idiopathic hyperostosis, massive ossification of the posterior longitudinal ligament and cervical and higher part of thoracic ligamentum flavum is reported in this article. Such comorbidity rarely occurs. Case Presentation: The patient’s chief complaint was sensation abnormalities without gait disturbance or ...
متن کاملCircumferential Spinal Cord Decompression through a Single Posterior Approach with Microendoscopy for Thoracic and Thoracolumbar Ossification of the Posterior Longitudinal Ligament
Thoracic and thoracolumbar ossification of the posterior longitudinal ligament (OPLL) can be difficult to treat due to the anatomical position. The purpose of this study was to report the significance of a novel surgical technique that represented two cases of thoracic or thoracolumbar OPLL. The first patient was a 72-year-old woman who had a beak-type OPLL at the T11/12. The second was a 45-ye...
متن کاملUse of an Ultrasonic Osteotome for Direct Removal of Beak-Type Ossification of Posterior Longitudinal Ligament in the Thoracic Spine.
Direct removal of beak-type ossification of posterior longitudinal ligament at thoracic spine (T-OPLL) is a challenging surgical technique due to the potential risk of neural injury. Slipping off the cutting surface of a high-speed drill may result in entrapment in neural structures, leading to serious complications. Removal of T-OPLL with an ultrasonic osteotome, utilizing back and forth micro...
متن کاملResults of surgical treatment for ossification of the posterior longitudinal ligament of the thoracic spine.
Conservative treatment is ineffective for ossification of the posterior longitudinal ligament (OPLL) in the thoracic spine, and surgical treatment is indicated for most cases, while such cases are not often experienced. In the present study, the results of surgical management involving mainly posterior decompression for this disease were evaluated clinically. The study included 9 patients (1 ma...

Ossification of the posterior longitudinal ligament and ossification of the yellow ligament are the main causes of spinal canal stenosis. This article describes a case of ossification of the posterior longitudinal and yellow ligaments on the lumbar spine. The patient presented with gradually worsening left lower-extremity ache and pain. The deep tendon reflex was hyperreflexia in the lower extr...
